The Canadian Forces Health Services

The history of the Canadian Forces Medical Service (CFMS) dates back to shortly after 1867 Confederation where Militia, with the inclusion of medical services, was formed to replace the British garrisons. The Canadian Army Medical Corps was formed in 1904. During the First World War the reserve component was organized as an Army Hospital Corps, with eight Cavalry Field Ambulances, fifteen Field Ambulances, and two Clearing Hospitals. In the Second World War we saw the expansion of the CFMS to include Casualty Clearing Stations, General Hospitals, and Convalescent Hospitals. Subsequent to the great wars, Canada furnished medical services to many UN and NATO missions. We are currently the medical lead nation to the effort in Afghanistan, with the provision of a Role 3 facility.

In the past few years the Canadian Forces Health Services Group has undergone a significant structural reorganization. Command and control was removed from the environmental commands and centralized nationally under a Health Services Command, with the realization of efficient health services and standardized training. The Branch continues to evolve as we set ourselves up for the future in a dynamic spectrum of military activity.

The current organization of the Health Services falls under the command of the Director General of Health Services who commands all aspects, both Medicine and Dentistry. The Surgeon General of Canada is the senior medical advisor on all medical clinical matters and the Director of Dental Services performs the same function for those matters on the Dental side of the business. The Reserve Field Ambulances fall under the command of one of the two Health Service Headquarters Detachments, as do the regular force units in a similar geographic area. Canadian Forces Health Services are currently organized into 14 Field Ambulances in the Reserve component and three in the Regular Force. The Reserve Field Ambulances are generally smaller than their Regular Force counterparts, but have the benefit of being situated in most major Canadian cities either as formed units or as detachments of larger units. Each Field Ambulance is commanded by a Lieutenant Colonel, and holds an approximate membership of 100 reservists. They recruit from the local community, and attract a wide spectrum of candidates; from 18-year-old (typically student) applicants, who possess no clinical training, to fully qualified paramedics, nurses and doctors who want to participate in the military.

The provision of medical support to similarly located Reserve military units undergoing training for operations is the primary mission of the Reserve Field Ambulance. The secondary role is to facilitate augmentation to the regular force for domestic and overseas operations. Due to the current reserve force unit size, capabilities and organizational establishments, augmentation of our Regular Force counterparts consists of individual soldiers and not entire units. Canada does not enact conscription, and hence, while the regular force may be compelled to go on tour, the reserve soldier can elect to participate.

Canada’s aim is to deploy approximately twenty percent of reserve soldiers for overseas tours. In Afghanistan, with a Role 1 and Role 3 medical force of approximately 200 personnel, this target equates to 20 medical staff. Since all soldiers who are selected for operational deployments undergo the same rigorous predeployment training and are also expected to follow the post-deployment follow-up activities, they must set aside 18 months of their time for this purpose. This includes our Medical and Dental Technicians, however, physicians and specialty nurses may deploy for as little as one month, to fill positions vacated by those deployed persons who are on leave. Additionally, our reservists fulfill numerous short-term domestic clinical and clerical positions by backfilling Regular Force training and deployment.

The whole Branch is currently undergoing a Field Force Review to look at all of our legacy structures and propose a plan to restructure them to better meet the needs of the modern battlefield. As change is directed and occurs, this website information will be updated.

Italian Medical Reserve Officers

RECRUITMENT RULES FOR THE ITALIAN MEDICAL RESERVE OFFICERS

In Italy there is an association of all the officers, included the medical ones.

It is possible to define two types of reserve medical officers:

  • Those who have completed their compulsory military service as officers;
  • Those who belong to the “selected reserve”, which is a novelty for the Italian army. The latter ones attend a formation course at the Scuola di applicazione in Turin; after that they receive the degree of officer of the selected reserve.

The service lasts from 45 days to 6 months. Of course, an officer can refuse the call.

There are also some voluntary medical officers who belong to the Italian military Red Cross who can be recalled as reservists.

Every armed force, like Army, Navy, Air force and Carabinieri, have its own medical officers.

Army Reserve

The Army Reserve is comprised of three groups of Soldiers

Selected Reserve
The Selected Reserve is the most readily available group of Army Reserve Soldiers. The Selected Reserve is comprised of Troop Program Units (TPUs), Active Guard and Reserve (AGR) Soldiers and Individual Mobilization Augmentees (IMAs). In the event of an emergency, all members of the Selected Reserve can be mobilized.

  • Troop Program Units (TPU)
    TPUs are the heart and soul of the Army Reserve. These men and women typically train on selected weekends and perform annual training.
  • Active Guard Reserve (AGR)
    AGR Soldiers serve full-time on Active Duty in units and organizations of the Army Reserve, or that directly support the Army Reserve.
  • Individual Mobilization Augmentees (IMA)
    The Army Reserve's IMAs are assigned to high-level headquarters where they would serve if mobilized. Most IMAs train annually for two weeks.

Individual Ready Reserve (IRR)
Members of the Individual Ready Reserve (IRR) are trained Soldiers who may be called upon, if needed, to replace Soldiers in Active Duty and Army Reserve units. Many of the Soldiers in the IRR have recently left Active Duty and still have an Army Reserve commitment. Others have chosen to remain Active as Army Reserve Soldiers but not as a unit member or IMA.

The Retired Reserve consists of retired Soldiers from the Army (Active Army, Army Reserve and Army National Guard) who remain part of the Army Reserve family.

Navy Reserve

The Navy Reserve Force consists of three categories. The "Ready Reserve" is made up of "Selected Reserve" (SELRES) personnel and "Individual Ready Reserve" (IRR) personnel. The SELRES is the Navy's primary source of immediate mobilization manpower and represents those Reservists who are paid, either as weekend drillers or who serve in Full-Time Support (FTS) on active duty status in the training and administration of the Navy Reserve Force program. Other reserve categories include the "Standby Reserve" and the "Retired Reserve" numbering over 690,000 men and women.

Air Force Reserve

Reserve Categories

Reservists are categorized by several criteria in the Ready Reserve, Standby Reserve or Retired Reserve. Numbers shown reflect actual numbers, not authorizations.

Ready Reserve
(Note: Counted Unit Program, Individual Ready Reserve, IMA and Selected Reserve)
The Ready Reserve is made up of 193,042 trained reservists who may be recalled to active duty to augment active forces in time of war or national emergency. Of this number, 72,195 reservists are members of the Selected Reserve who train regularly and are paid for their participation in unit or individual programs.

These reservists are combat ready and can deploy to anywhere in the world in 72 hours. Additionally, 48,981 are part of the Individual Ready Reserve. Members of the IRR continue to have a service obligation, but do not train and are not paid. They are subject to recall if needed. The president may recall Ready Reserve personnel from all Department of Defense components for up to 270 days if necessary. Some 24,000 Air Force reservists from 220 units were called to active duty during the Persian Gulf War to work side-by-side with their active-duty counterparts.

Standby Reserve
The Standby Reserve includes reservists whose civilian jobs are considered key to national defense, or who have temporary disability or personal hardship. Most standby reservists do not train and are not assigned to units. There are 16,858 reservists in this category.

Retired Reserve
The Retired Reserve is made up of officers and enlisted personnel who receive pay after retiring from active duty or from the Reserve, or are reservists awaiting retirement pay at age 60.

Military Medical Association of the Macedonian Reserve Officers
Section of Medical Reserve Officers
Founded in accordance with the Statute of ORO RM
As proficient association for improving military medicine

The seat of the Section is within the headquarters of ORO RM.

The Section encompass:

  • Leadership at state level and
  • Leadership branches at the municipality organizational level.

Responsibilities:

  • supports organization and preparedness of the medicall service within national and international framework;
  • promotes standardization and interoperability among medical units of different levels; improves efficiency medical services;
  • contribute to the development and mutual understanding among active and reserve military medical forces;
  • establishes cooperation with similar national and international organizations; provides exchange of information;
  • organizes expert and scientific meetings, courses and training programs within the frames of ORO of RM and within the working environment.

The Section of reserve medical officers
May become all members of ORO-RM who have completed medical, dental, pharmaceutical or veterinary education (from 4th to 8th degree).

Membership is a personal legacy and moral act without considering the age and is not related to the constitutional and legal obligation for engagement in case of disasters and crise situations or war ( which understands deployment within the working organization or deployment of forces for the protection and defending the territory or deployment within the
security forces).

Membership is acquired with submission of a special membership application within the correspondent municipality organization of ORO of RM, after the acceptance of the application.

All members bear the costs for the annual participation of collective membership within: The International Confederation of medical reserve officers in NATO (CIOMR). Section provisions predict cases and situations when members may lose their membership legacy. POC: 02/2454 376 & 070/64 80 64.
